Community Nursing
Depending on the kind of disability, participants require various kinds of medical care and attention.
The community nursing care program by Diverse Disability Services prescribes the need for participants to become independent when it comes to their health, as a major part of the program. Through the community nursing care program, participants are trained on how to take care of their health in scenarios such as medication, wound care, hygiene, and help with showering or dressing. There is usually a registered nurse or medical practitioner appointed who will monitor the patient’s progress and will amend the treatment and medication accordingly.
Here are some of the services offered under the community nursing care program:
- Visiting nurse and assessment
- Developing treatment plans
- Case management
- Setting up medication
- Medication administration/supervision
- Wound care
- Stoma care
- Respiratory support
- Continence advice and management
- Palliative support
- Diabetes management
- Dementia management
- Hospital stay after-care
- Overnight nursing care
- Monitoring of vitals
- Education for participants and their families
At Diverse Disability Services we have certified and experienced medical professionals who can render community nursing care under the NDIS program. We provide an obligation-free consultation in the comfort of your own home, with the purpose of listening and understanding your needs.
With us, each participant that has community nursing care in their NDIS support plan is allocated a medical staff member who will keep in touch to discuss, monitor, and provide feedback on the participant’s medication, treatments, and progress. Besides this, our medical staff members offer advice on how to engage in basic self-care and nursing wherein one will become capable of doing simple things like wound care or first aid on their own.

Supported Independent Living (SIL)
Diverse Disability Services is a provider based in Brisbane and is committed to providing premium quality supported independent living arrangements (SIL).
We pride ourselves on catering to the individual needs of people with a range of disabilities and crafting homes where people can enjoy a fulfilling and happy life.
Supported Independent Living (SIL) is help with and/or supervision of daily tasks to develop the skills of an individual to live as independently as possible. These are the supports provided to a participant in their home, regardless of property ownership, and can be in a shared or individual arrangement.
Assistance provided to a participant will be included as part of their plan depending on the level of support they require to live independently in the housing option of their choice.
SDA Housing is uniquely designed to improve liveability, accessibility, freedom and independence through the inclusion of purpose-built house modifications, the consideration of optimal location, the robust and secure environment that prioritize safety, and through the high level of specialized care provided within the homes.
What is SDA in NDIS?
Some NDIS participants receive funding for Specialist Disability Accommodation (SDA). SDA has accessible features to help residents live more independently and have better access to the supports in their home. SDA funding in participant plans is used for housing, not services or support.
Specialist Disability Accommodation (SDA) is provided within four separate categories: Improved Livability, Fully Accessible, High Physical Support, and Robust. Diverse Disability Services is a provider for all SDA categories.
What is the SDA standard?
The SDA design standard improves clarity about design requirements, provider confidence and compliance by giving: participants access to high-quality housing models. providers flexibility to respond to participant needs and preferences.
What type of SDA funding can I access?
When a participant is eligible for SDA, the NDIA will provide that participant with an amount of funding. The NDIA considers the participant’s goals and preferences, and calculates the amount of SDA funding based on:
- Design category – determined by the participant’s support needs
- Dwelling type – determined by both the goals and needs of the participant to achieve those goals
- Location – determined by both the goals and needs of the participant, and which represents value for money.
The participant can then use that funding to find a dwelling to move into that is consistent with their goals and support needs. SDA funding is provided to a participant, not a dwelling, and a participant can move from one dwelling to another.
